Issue
After the Oracle database has had an unexpected failure which was subsequently fixed, the RSA Identity Governance & Lifecycle application remains running and was never shut down. The user interface may or may not be available. If this is a cluster, one or more nodes may be unresponsive. If the user interface is accessible and collections are run, they may get stuck.
 
Cause
When the database unexpectedly fails, RSA Identity Governance & Lifecycle does not shutdown automatically but goes into an inconsistent state.
 
Resolution
Restart AFX, RSA Identity Governance & Lifecycle and the Oracle database. Even if the RSA Identity Governance & Lifecycle application appears to be working correctly, all three applications should be restarted. 

Login as user oracle and execute the following commands:
 
afx stop
acm stop
acm stopdb
acm startdb
acm start
afx start
