RSA Identity Governance and Lifecycle Change Request created without required Create Account item

Issue
The following steps will reproduce the issue:
- Create an explicit access request.
- Select an entitlement from an application that requires an account.
- Move to the submission page.
- Click back and forth between the submission page and the previous page.
- Finally, submit the request.
- You will see the request does not contain the Create account item, though the local account is created in application/directory.
- Due to this, the CR will remain stuck in the Wait for dependency state.
