CRs created from Role Rules that include account creations create pending accounts with wrong Account Names in RSA Identity Governance and Lifecycle

Issue
When running Role-Rules (UINC or Missing Entitlements Roles), the CR (sometimes) gets stuck in open state. All users have this attribute populated in the account template, however the "Create Account" ignores the account template settings and creates the CR with wrong name in the format "<UserID> account on <App Name>" for some (sometimes most or all) accounts being created.

The following is an example of where the request was canceled. Notice the pattern on line "P4-Guest account on Perforce" where in this example P4-Guest is the account/UserID and the application name is Perforce.

Resolution
This issue has been resolved in version 7.0.1 P02.
