Issue
For any change request raised via Webservices, when clicking on the form under Additional Information, the following error appears:

An error occurred loading the fields for the form

Cause
This is a bug fixed in RSA Identity Governance & Lifecycle 7.0.2 P07 and 7.1.0 P01
Resolution
Deploy patch 7.0.2 P07 or 7.1.0 P01, respectively to have this issue fixed.
