Issue
We have the email to approve or reject request. Sometimes, the processing Result is 'Item was already closed', while the request was actually still wait for the approval. Thus, subsequent approval email sent won't work. 
Resolution
As per our previous communication, I have already submitted this issue as RFE (https://community.rsa.com/ideas/2603) and engineering has confirmed that this will be treated as RFE.  

Engineering has also mentioned that:
For the case of a browser being closed, we will possibly introduce a functionality that will try to identify the work items opened on a older date than today and put such approvals back in the open pool and present better error message for already closed items.  This will allow to approve an item if previous approver closed the browser without Approving/Rejecting an item which results into the open approval.

This will be added in the future release and will be treated as an enhancement request. 

The more votes this RFE gets, higher the chances that enhancement will be considered by Product Management Team.
